Investigative Reports
Social Security Reports [SSN Report]
This report should start your search for information on your client. It will provide you with all known names and addresses linked to your client's social security number, spanning as far back as the history of the number. You will discover where they have lived, when they lived there, and any additional dates of birth associated with this SSN. It also lists the date of death. This report does not include protected health information, and an authorization is not required. Pay for this report via the PayPal button and send seperate email to:cynthia@medical-eyes.net with client name and all known SSNs.
Physician Finder Report [PFR Report]
Once you know where your client has lived, and have correct dates of birth and death, you can order the PFR report. This report provides a comprehensive 5-year history on your client's prescription drug use. It provides complete pharmaceutical detail, including drugs prescribed, prescribing physicians and dispensing pharmacies, and complete address information for both physicians and pharmacies. This is an affordable, valuable tool for medical malpractice, toxic tort, personal injury, and workers' compensation cases! Use it to uncover likely diagnoses and likely pre-existing medical conditions, and to analyze “physician hopping,” or “drug seeking” behavior. This report includes protected health information and requires a HIPAA compliant authorization, which we can provide for your client to complete.
Hospital Finder Report [HFR Report]
This report is a telephone canvassing report. When you order this report, we will generate a SSN Report (above) to provide you with known addresses for your client. You then may choose one or several locations. Based upon your choices, we will call a minimum of eight Hospitals in each area, and ask non-HIPAA related questions to determine if your client was ever admitted to that hospital, and if that hospital has medical records that you can obtain. This report is ideal for cases that have been difficult in discovery and for which you need to uncover more medical information. This report does not include protected health information, and no HIPAA compliant authorization is required.
